This decision is final unless appealed within 30 days of the mail date of this notice. Appeals must be in writing and be sent to the field office listed on this notice. The Appeal Form (DE 1000M) is available on our website at edd.ca.gov/pdf_pub_ctr/de1000m.pdf. In your appeal you must state the reasons why you do not agree with this decision.
When your appeal is received, your case will be reviewed. If the decision remains the same, we will send your appeal to the Office of Appeals. If you appeal after the 30 days, you must include the reason for the delay. The Administrative Law Judge will determine whether you had good cause for the delay.
