INSTRUCTIONS FOR MILITARY AND FEDERAL CLAIMS

You have filed a claim for unemployment insurance (UI) based on wages earned as an ex-service member (UCX) or as an ex-federal civilian employee (UCFE). Please follow the instructions checked below to complete the claim filing process. It is important to submit your bi-weekly certification for Unemployment Insurance benefits while you are waiting for the wages to be added to your claim.

EX-FEDERAL CIVILIAN EMPLOYEES (UCFE)

On the ETA 935, Claimant’s Affidavit of Federal Civilian Service, Wages, and Reason for Separation form enclosed, please complete items 9a through 9e and sign and date in item 10 of the form. Return the form to the above EDD address with either the SF 50 or copies of your check stubs dated between MM/DD/YYYY and MM/DD/YYYY. Do not send original documents. BENEFITS CANNOT BE PAID UNTIL WAGE INFORMATION IS RECEIVED.

EX-SERVICE MEMBER (UCX)

Please send a copy of your DD 214 (Member 4) to complete the claim filing process to the above EDD address. The DD 214 and wages information has not been received from your Military Branch of Service. Do not send original documents. BENEFITS CANNOT BE PAID UNTIL THE DD 214 (Member 4) IS RECEIVED.

On the MA 8-43, Request for Military Document/Information for Unemployment Compensation Purposes (UCX) form enclosed, please sign and date the form at the bottom of section II and review item # 16 for any requested documents or information. Send the MA 8-43 form with copies of any proof documents to the Military Branch of Service listed below. Do not send original documents. This additional information is needed to clarify the information on your DD 214 to complete the claim filing process or you have indicated that you do not have a copy of your DD 214 (Member 4) and wish to request one.
