The Employment Development Department (EDD) is unable to process the California Training Benefits (CTB) Application and School and Training Questionnaire, DE 4365TQ, that you submitted to determine your eligibility for unemployment insurance (UI) benefits and the CTB program while you attend school or training for the reasons indicated on this notice. Please follow the instructions checked below. Failure to do so may result in a further delay or denial of your UI benefits.

Your questionnaire, DE 4365TQ, is being returned to you because you did not provide answers to all the questions, the responses were illegible, or there was conflicting information. The EDD cannot determine your eligibility for UI benefits and the CTB program without this information and UI benefits cannot be paid. The items that need to be completed or corrected on the questionnaire have been highlighted.
Please complete or correct the highlighted items on the enclosed questionnaire and return it within 10 calendar days from the mail date of this notice. You can fax the completed questionnaire to ‎1-855-873-4359 or mail the completed form to the Employment Development Department, P.O. Box 599000, Elk Grove, CA 95759 in the enclosed envelope.

Your corrected questionnaire must be returned to EDD within ten (10) calendar days or your eligibility to receive UI benefits will be based on available information, which may result in a denial of benefits. Please write your name and Social Security number (SSN) on any additional documents you include to establish your enrollment and attendance in the school or training program that you reported to EDD . If you have questions about the questionnaire or need assistance, you can contact the EDD by going online at www.edd.ca.gov and clicking the “Contact EDD” link or you may call one of the toll-free numbers listed at the top of this notice.
Based on the information you provided on the questionnaire, you are not currently attending school or training. Your eligibility for UI benefits and the CTB program cannot be determined until after you begin attending school or training. You must report the start of school or training in the week it actually begins when you certify for benefits for that week. After you report the start of school or training, you will be scheduled for a telephone interview to determine your eligibility for UI benefits and the CTB program while you attend school or training.

You have not responded to the Additional Instructions, DE 238, mailed to you on Blank line, requesting you to provide proof of wages to determine your eligibility to receive benefits on your potentially valid new claim beginning Blank line. UI benefits cannot be paid until you provide proof to the EDD as instructed that you earned the required amount of wages and you are determined to be otherwise eligible to claim UI benefits. After you provide the requested information and your UI claim is potentially payable, you will be scheduled for a telephone interview to determine your eligibility for UI benefits and the CTB program while you attend school or training.

Your claim is currently invalid and pending receipt of documents to verify your identity as requested on the Request for Identity Verification, DE 1326C, mailed to you on Blank line. The EDD cannot determine your eligibility for UI benefits and the CTB program until you provide the requested documents to the EDD as instructed and you are determined to be otherwise eligible to claim UI benefits.
Your claim is currently invalid and pending the recalculation of your claim award after wages are added to the base period of your claim beginning Blank line. Benefits cannot be paid until the wages are added, your claim becomes valid, and you are determined to be otherwise eligible to receive UI benefits. Once your claim is valid, you will be mailed a notice of your claim award and your eligibility for UI benefits and the CTB program while you are attending school or training can be determined.
