Instructions for Relocation Allowances

Trade Act of 1974, as Amended

When your relocation has been completed, you are to submit a final statement covering the following:
  1. Names, dates, and mode of transportation of each member of your family who actually relocated. (When a family member who plans to move later than the other members of your family has actually relocated, a final statement regarding such family member’s travel will also be submitted.) A relocation will be completed when you and your family and your household goods and personal effects have actually been moved to the area of relocation and there established a residence.
  2. Actual cost of transporting your household goods. Upon completion of the transportation of household goods, you must promptly submit:
    1. If by commercial carrier, the original or a certified copy of the bill of lading prepared by the carrier, including a receipt evidencing payment of the transportation cost. In addition, you are required to submit with the bill of lading a check covering the amount, if any, by which the advance, made to you for such transportation cost, was in excess of the actual amount charged for transportation. (If the cost of transporting your household goods was paid to the carrier by the State, this instruction does not apply to you.)
    2. If the trailer is hauled by commercial carrier, a receipted bill from the carrier evidencing payment of the actual hauling charges. In addition, you are required to submit with the receipted bill, a check covering the amount, if any, by which the advance made to you for such transportation cost was in excess of the actual amount charged for transportation.
    3. If by a rented trailer, a receipted bill itemizing, and evidencing payment of, the actual rental charges for the trailer. In addition, you are required to submit with the receipted bill, a check covering the amount, if any, by which the advance made to you for such transportation cost was in excess of the actual amount charged for transportation.
  3. The date you actually entered employment with the employer shown on DE 8321 part A. 7., "Starting Date." If you failed to enter employment with such employer, state the reasons for your failure to enter such employment. If you are employed with another firms, show the employer’s name, address, and date entered such employment.
    If your relocation will not begin within 182 days of the date of your application, you must submit the reason(s) in writing to:
