The Employment Development Department (EDD) is conducting a Benefit Accuracy Measurement (BAM) Audit to determine if Unemployment Insurance benefits are properly paid. Your Unemployment Insurance claim has been randomly selected by the Department of Labor (DOL) for this review.
A BAM Analyst will be contacting you by telephone to obtain information concerning your Unemployment Insurance claim and to gather information about your employment history for the last two years. This audit process will also include: verification of monetary eligibility; employment separation; wages and other earned income; review of prior eligibility determinations; review of availability/ability to work and refusal of work.
To assist you in preparing for this interview, we have enclosed an Unemployment Insurance BAM Claimant Questionnaire for your review. Please have this form in front of you when we call and be prepared to provide your answers to each of the questions.
If you have returned to work, please call the above number immediately and complete the following information and the Unemployment Insurance BAM Claimant Questionnaire and return it in the enclosed envelope.
